Aged Care Partner Hub

Whether you're putting a Support at Home package together, reviewing one, or working out whether allied health belongs in it, this is the place to start. Take the AT and Home Modifications Guide, or claim an early access place for the BlueRocket Connect care manager log-in. Both are free.

The Assistive Technology and Home Mods Support at Home Guide

An equipment or home modification request usually arrives with a deadline and a question underneath it: who has to assess this, and what does the approval need to see? The Guide is a printable checklist for exactly that moment. Pick the kind of item and it tells you the type of evidence the request typically needs.

Who is qualified to prescribe it

For each kind of item or modification, whose assessment the request needs, so you know who to involve before the clock starts.

What the approval needs to see

The documentation an approval typically asks for, laid out as a checklist you can tick off before anything is submitted.

Why requests bounce back

The things that most often send a request back for more information, so they're covered the first time.

How long things realistically take

Realistic lead times from request to item, so you can set expectations with a client and their family early.

Early access to BlueRocket Connect

BlueRocket Connect is the portal we're rolling out to BlueRocket families in September — appointments, session summaries, therapy goals, documents, clinical resources and funding information, all in one place.

From 19 October, we're opening BlueRocket Connect to the Aged Care Service Coordinators we work with too, so you can see the same picture for the participants you coordinate, instead of digging back through old emails every time.

We're offering early access to only 20 Coordinators in our network. That's how many we can onboard properly and genuinely listen to while BlueRocket Connect is still taking shape.

What you'll be able to do in BlueRocket Connect

Icon

See everything in one place

What's been happening at sessions, the goals being worked on and the strategies being used, therapy documents to download, and how much funding has gone against the plan.

Icon

Stay in the loop automatically

Get an update the moment a session is completed, a report is finished, or a document is added — instead of waiting to hear.

Icon

Ask a question, skip the email chain

Post a question about a participant for their treating therapist to pick up at the next session, rather than starting an email chain.

Ask LittleRocket, your AI assistant

Ask in plain English and it pulls the answer from everything above — sessions, goals, documents, funding. It only draws on what's already in the participant's record, and never gives therapy advice.

You may enjoy BlueRocket Connect if…

  • You share at least one participant with a BlueRocket clinician
  • You like having what you need in front of you before a plan review.
  • You're often the person a family asks what's happening with therapy
  • You're happy to tell us what's working well and what’s missing
