The Assistive Technology and Home Mods Support at Home Guide
An equipment or home modification request usually arrives with a deadline and a question underneath it: who has to assess this, and what does the approval need to see? The Guide is a printable checklist for exactly that moment. Pick the kind of item and it tells you the type of evidence the request typically needs.
