In-Home Functional Capacity Assessments in Perth
BlueRocket provides Functional Capacity Assessments to NDIS clients of all ages in the Perth Metro area.

How our in-home Functional Capacity Assessments work
A Functional Capacity Assessment in Perth (FCA) is conducted by a qualified Occupational Therapist (OT) to understand how you manage everyday activities and which supports may help you live more independently. The assessment component of an FCA takes place in your home so your therapist can observe how you navigate real environments and routines.
Through a combination of consultation, observation and structured assessment, our BlueRocket OTs build a clear picture of your strengths, challenges and current functional capacity. This allows our team to identify the level and type of support that may improve your participation in daily life.
Our recommendations are then compiled into a detailed FCA report, which can be used to support NDIS planning decisions, funding requests and access to appropriate services and supports.

What's included in our in-home Functional Capacity Assessments
BlueRocket’s Functional Capacity Assessments in Perth help NDIS participants understand their current abilities and identify the supports needed to improve independence and make everyday tasks easier. Each assessment builds a detailed picture of strengths and challenges, with tailored recommendations to inform funding and support planning. Assessments typically include:
- Observation of daily tasks and routines in the home, school or workplace.
- Use of tools and techniques to assess physical, cognitive and psychosocial abilities.
- Evaluation of communication, mobility, self-care and household activities.
- Tailored recommendations for support hours, therapy goals and participation strategies.
- Trialling, fitting and training for Assistive Technology - e.g. wheelchairs and eye-tracking systems.
- Recommending Home Modifications - e.g. ramps and grab rails.
Steps Involved
BlueRocket FCAs take a structured, client-focused approach. Our process provides a clear understanding of your strengths, support needs and participation goals.
Initial assessment & goal planning
Your assessment starts with a conversation in your home to gain insight into your abilities, challenges and goals. Working alongside your assessor, you’ll determine which areas of daily life to assess, including self-care, household tasks, communication and community engagement. This creates the foundation for your personalised FCA plan and informs future recommendations and NDIS planning.
Observation & assessment
During the FCA, your assessor will observe you performing everyday tasks in relevant environments, such as your home, school or community settings. This step identifies strengths, limitations and areas where additional support could improve independence and participation.
Specialised, multi-disciplinary supports
We collaborate seamlessly with Speech Pathologists and Occupational Therapists to holistically meet your needs. Where needed, we provide:
- Assistive Technology assessment and prescription (e.g. mobility aids, wheelchairs).
- Home Modifications recommendations (e.g. ramps, grab rails).
- Early Childhood Intervention for developmental motor skills.
Reporting, review & adaptation
Following the assessment, a detailed report is provided that clearly outlines your strengths, areas for support and recommended strategies. We also include guidance for NDIS planning and ongoing service decisions. Our team ensures that you and your family fully understand our findings, giving you the clarity to take your next steps comfortably. Plans can be reviewed as needed to ensure they remain relevant and aligned with your evolving goals.
Who We Support
An FCA gives a clear picture of an NDIS participant’s current abilities and support requirements. It’s a helpful first step toward greater independence and participation, providing personalised recommendations alongside the evidence needed for NDIS funding decisions. FCAs are particularly useful for:
- Individuals applying for NDIS funding for the first time – an FCA outlines your support needs in detail and offers a structured plan to help you start your journey confidently.
- Existing NDIS participants preparing for a plan review – if your circumstances or goals have changed, an FCA can reassess your needs and provide updated recommendations, including guidance on any additional funding that may support your progress.
- Participants undergoing an NDIS eligibility reassessment – an FCA can provide supporting information to help ensure your plan accurately reflects your current needs.
It’s important to note that an FCA isn’t a requirement of the NDIS. You can also apply for funding with a letter from a health professional.
Specialised Services
BlueRocket FCAs can also link you with a range of specialised supports that may be recommended as part of your assessment outcomes. As part of a holistic care plan, these services focus on improving everyday functioning, supporting practical outcomes and providing clear strategies to help you achieve your goals.
